Managers are encouraged to complete this form 1-2 months into an internship and review with their intern.

Questions

  • What progress is the intern making towards meeting the SMART goals they established?
  • What do you perceive to be the intern/s greatest strengths? If possible, give specific examples/stories to illustrate your intern’s strengths.
  • What areas of growth could improve your intern’s success at your workplace?  If possible, give specific examples to illustrate your points.
