Program Manager, New York City Health Hospital

So, my name is Hilla Reyes. I'm the program manager for the asthma program. I work for New York City Health and Hospital especially the location is Lincoln here in the Bronx. So, as a program manager, I supervise a group of six community health workers and the asthma program. So, my duties is make sure that they are up to date with the case load, you know, health meetings, meet with the clinical staff, also at the end of the fiscal year make sure that the metrics are met. We do outreach over the phone to see how they doing with the asthma. We do education about the asthma. Some patients, they don't know how to take the medication. Most of them, they're not connected with the primary care provider or with the pulmonary clinic. So, we basically connect patients with the medical team. The CHW also do home visits. They go to the houses and see the home environmental issues that trigger the asthma. We also send a report to the Department of Health so they can do an inspection and see how the situation is at home so they can fumigate and see what is triggering the asthma. My biggest responsibilities is make sure that each of the CHW are on track of the assessment of the patients. They have a caseload, specifically 30 patients that they need to do a specific assessment: first assessment, three months assessment and six months assessment. So, I have to make sure that they're up to date by the specific times. At the end of the fiscal year when I was recently hired, I was at the end of the fiscal year. So, we have to report back to our supervisors the metrics that the past program manager did in the program. So, I have to review that the data entry was correct that the numbers of patients that we report that they were closed and patients enroll match with the data that they report. So, I have to go back, review, make sure that the data entry was correct.
